Please print, complete, and sign ALL the REQUIRED forms below and bring them with you to your first appointment! Please note: We require 48 hours notice for new patients who wish to cancel or reschedule their first appointment.
Please arrive at least 10 minutes early to your appointment with completed paperwork. You can also fax your completed paperwork prior to your appointment. If you have not completed your paperwork, please arrive at least 30 minutes early.
To provide all her patients exceptional care and individualized attention, Dr. Retzler does not double book appointments. If you do not have your paperwork completed at the time of your appointment, your appointent might be cut short or you may have to be rescheduled.
